Free-tier requests stay on free models

Free-plan requests are no longer silently served by premium models when a free model is temporarily unavailable — your plan's model list is now respected through every fallback step. If no free model can serve the request, you get a normal try-again error instead. Paid plans are unaffected and keep full fallback across the whole catalog.
